Special Lecture: Aptitude à la conduite: quelles sont les nouveautés 2016 ? L’année 2016 voit arriver une révision de l’Ordonnance réglant l’admission à la circulation routière (OAC) qui concerne particulièrement les médecins amenés à effectuer les examens d’aptitude à la conduite automobile, adoptant un système progressif d’exigences applicables à ces médecins. Les nouvelles dispositions ont pour but d’améliorer la qualité des évaluations de l’aptitude à la conduite. De plus, elles permettent d’adapter les exigences médicales minimales imposées aux conducteurs, lesquelles datent des années 70, à l’état actuel de la science et de la technique. Nous aurons aussi l’occasion de décortiquer les directives relatives aux conducteurs souffrant de diabète qui ont récemment été révisées.

Gastgesellschaft / Société conviée: SGPMR / SSMPR: Translation von Basiswissenschaften zu Rehabilitation Malgré les nouveaux traitements la sclérose en plaques (SEP) reste une maladie invalidante entraînant progressivement une perte de la mobilité. Face à une telle évolution le challenge du thérapeute est multiple: il doit proposer les moyens auxiliaires pour limiter la fatigue, trouver les mots pour encourager son patient à faire des exercices afin de maintenir sa mobilité résiduelle ou à l‘inverse lui offrir des traitements qui réduisent ses spasmes douloureux liés à l’immobilité. Et finalement il lui faudra savoir utiliser les échelles de mobilité adéquates pour prouver aux assurances que son travail est efficace! La clinique bernoise de Montana qui accueille chaque année plus de 300 patients souffrant de SEP a le privilège de pouvoir s’appuyer sur une expérience d’un demisiècle dans le domaine de la rééducation de cette très particulière maladie et je me réjouis de partager dans cet exposé quelques expériences faites dans notre établissement : Etant donné que l’échelle de Kurtzke EDSS (Expanded Disability Status Scale) considérée comme le « gold standard » pour évaluer le handicap des personnes SEP s’est avérée pas suffisamment précise pour les personnes en rééducation et donc plus fortement atteintes dans leur mobilité, nous avons validé d’autres outils tel le RMI (Rivermead Mobility Index) 1 et nous en avons développé un autre, sous forme d’application pour Iphone et Ipad, le SaGAS (Short and Graphic Ability Score) 2. Pour calmer les spasmes nous avons été par les premiers à offrir à nos malades des extraits de cannabis et nous avons pu constater – outre l’analgésie procurée que ce traitement n’allait pas réduire leur mobilité, mais bien au contraire leur en faire gagner.3 Comme d’autres cliniques nous avons aussi voulu faire travailler nos malades SEP avec le robot de marche Lokomat. Cette expérience nous appris qu’il était tout aussi efficace de faire marcher les patients dans une salle de gym que de les « enchaîner » sur un Lokomat. 4 1. Gastgesellschaft / Société conviée: SAPPM / ASMPP: Wie die Stressverarbeitung im Gehirn Schmerzen machen kann – und wie man solche Schmerzen heilen kann Die Schmerzforschung der letzten Jahren hat gezeigt, dass bei vielen chronischen Schmerzstörungen Ausmaß und Stärke des individuell erlebten Schmerzes nicht mit dem Ausmaß der peripheren Gewebeschädigung oder der Aktivität der primärafferenten und der spinal-nozizeptiven neuronalen Bahnen korrelieren. Bei einigen chronischen Schmerzstörungen, z.B. Fibromyalgie-Syndrom, craniomandibuläre Dysfuktion (CMD) oder somatoforme Schmerzstörung, ist überhaupt kein relevanter peripherer Input nachweisbar, vielmehr wird der Schmerz durch neurobiologische Mechanismen zwischen Schmerz- und Stressverarbeitungssystem in verschiedenen Hirnbereichen sowie über neuroinflammatorische Prozesse generiert („Stress-induzierte Hyperalgesie“), denen Prägungen in der Kindheitsentwicklung zugrunde liegen. Daraus leiten sich spezifische nicht-medikamentöse Therapiestrategien ab, welche solche Schmerzen vollständig und anhaltend zum Verschwinden bringen können. Learning Center: Notfallechokardiografie für die ICU Notfallechokardiographie für die ICU: In diesem Workshop wird die echokardiographische Abklärung der fünf häufigsten Fragestellungen in der Akutmedizin zum Herzen auf möglichst einfache Weise vorgestellt: Akute Rechtsherzbelastung, Perikarderguss / Herzbeuteltamponade, Bestimmung der linksventrikulären EF (biplan nach Simpson), Quantifizierung einer Mitralklappen-Insuffizienz und Aortenklappenstenose. Die Durchführung wird zunächst theoretisch kurz vorgestellt, danach in Videobeispielen anschaulich erläutert und mit praktischen Übungen die 3D-Orientierung in den relevanten Standardebenen vertieft. Anschließend besteht für Interessierte auch die Möglichkeit, die echokardiographischen Techniken auch selbst praktisch am Sonogerät unter Anleitung durchzuführen.
