MAIL SERVICE PROGRAM OPTIONS Your pharmacy benefit plan has enrolled you in the Mail Service Member SelectSM program. This program will help you better manage the medication that you take on a regular basis and may save you time and money. The Mail Service Member Select program allows up to two fills of your maintenance medication at a retail pharmacy. All additional fills may be at the full out-of-network cost. To avoid paying more, you can transfer your prescription to the OptumRx® Mail Service Pharmacy or you can disenroll from mail service and continue to fill your medication at a retail pharmacy for your standard co-pay or cost. If you prefer to disenroll from mail service, log in to myuhc.com®, select Manage My Prescriptions and then follow the steps outlined below. If you prefer, you may call the member phone number listed on the back of your health plan ID card to disenroll from Mail Service Member Select.
Select My Account from the top navigation bar.
2 Select Manage Account from the subheading found under My Account.

Locate the Manage My Mail Service Options to update your Mail Service Member Select program preference.
4 Select the DISENROLL button to opt yourself out of the Mail Service Member Select program.
5 Once you disenroll from the Mail Service Member Select program, you will see a pop up confirming your decision.
6 After one year, if you still do not wish to participate in the program, you will need to renew your disenrollment decision by selecting the EXTEND button.
